Privacy Policy

Your Privacy, Our Priority.

1.1 Introduction

Care Concierge Inc., doing business as Care Concierge ("Care Concierge," "we," "us," or "our"), respects the privacy of prospective clients, clients, responsible parties, family members, caregivers, job applicants, referral sources, website visitors, and others who interact with us. This Privacy Policy explains how we collect, use, disclose, protect, and retain information obtained through our website, online forms, telephone communications, email, text messaging, electronic systems, and related digital services. It applies to information collected through the website and related online interactions. Separate service agreements, client notices, employee policies, and legally required health-information notices may also apply.

1.2 Information We May Collect

We may collect information you voluntarily provide, information generated through your use of our website, and information received from authorized third parties. Depending on the interaction, this may include:

  • Identity and contact information: name, mailing address, email address, telephone number, date of birth, and information about a responsible party or authorized representative.

  • Care inquiry information: requested services, scheduling needs, service location, functional limitations, mobility or transfer needs, personal-care needs, household support needs, safety concerns, and other information needed to evaluate whether Care Concierge can provide appropriate non-skilled services.

  • Health-related information: information voluntarily provided about diagnoses, medications, treatments, allergies, medical providers, equipment, conditions, or other health matters when relevant to a care inquiry or service plan. Submission through a general website form does not by itself establish a client relationship.

  • Payment and billing information: billing address, transaction details, payer information, and limited payment information. Payment-card data may be processed by third-party payment processors rather than stored directly by Care Concierge.

  • Communications: emails, text messages, call details, voicemail, form submissions, complaints, feedback, and records of communications with us.

  • Website and device information: IP address, browser type, device type, pages visited, referring pages, approximate location derived from IP address, cookie identifiers, and similar technical information, subject to the technologies actually deployed on the site.

  • Employment or contractor information: information submitted by applicants or prospective workers, including work history, qualifications, credentials, references, and other application information.

1.3 How We Use Information

We may use information for legitimate business, care-coordination, regulatory, safety, and administrative purposes, including to:

  • respond to inquiries and determine whether requested services are within our licensed and operational scope;

  • communicate with prospective clients, clients, responsible parties, caregivers, referral sources, and applicants;

  • schedule assessments, consultations, services, supervisory visits, or other appointments;

  • create and maintain service agreements, service plans, client records, documentation, billing records, complaint records, and quality-improvement records when applicable;

  • coordinate authorized non-skilled care with family members, responsible parties, facilities, physicians, or other providers when permitted or authorized;

  • process payments and manage accounts;

  • recruit, screen, onboard, schedule, and communicate with workforce members and applicants;

  • operate, secure, troubleshoot, and improve our website, CRM, forms, communications, and business systems;

  • prevent fraud, misuse, security incidents, and unlawful activity; and

  • comply with Georgia Private Home Care Provider requirements, subpoenas, court orders, licensing inspections, reporting duties, and other applicable law.

1.4 Health Information, HIPAA, and Georgia Client Confidentiality

Care Concierge provides non-skilled private home care services and may receive sensitive health-related information. Georgia Private Home Care Provider rules require the agency to maintain the confidentiality and security of client records and limit disclosures of client-record information. Care Concierge will handle client information in accordance with those requirements and other applicable law.

HIPAA does not apply to every organization that handles health information. A health care provider generally becomes a HIPAA covered entity when it transmits health information electronically in connection with a transaction for which HHS has adopted a standard. Accordingly, Care Concierge will comply with HIPAA and its Privacy, Security, and Breach Notification requirements to the extent those laws apply to Care Concierge's operations. Nothing in this Privacy Policy should be read as a representation that every website interaction is governed by HIPAA.

If Care Concierge becomes subject to HIPAA as a covered entity, it will provide any legally required Notice of Privacy Practices separately. If a technology vendor creates, receives, maintains, or transmits protected health information on behalf of Care Concierge in circumstances requiring a Business Associate Agreement, Care Concierge will use an appropriate written agreement before permitting that access.

1.5 When We May Share Information

We do not sell client medical or care records. We may disclose information only as reasonably necessary and permitted by applicable law, including to:

  • Authorized Care Concierge personnel who need the information to perform their duties.

  • The client, responsible party, or authorized representative, as appropriate.

  • Health care providers or facilities when authorized, required, or reasonably necessary for permitted care coordination and consistent with applicable privacy law.

  • Service providers that support hosting, CRM, communications, scheduling, document management, payments, cybersecurity, analytics, or other operations, subject to appropriate contractual and privacy safeguards.

  • Government and regulatory authorities, including the Georgia Department of Community Health, when access or disclosure is authorized or required for licensing, inspection, investigation, reporting, or legal compliance.

  • Law enforcement, courts, or other parties when required by subpoena, court order, applicable law, or to protect rights, safety, and security.

  • Successors or transaction parties in a merger, acquisition, reorganization, financing, or sale of business assets, subject to applicable confidentiality and legal obligations.

1.6 Cookies, Analytics, and Tracking Technologies

Our website may use cookies and similar technologies for essential functionality, security, preferences, performance measurement, and analytics. Care Concierge should configure tracking technologies conservatively because visitors may reveal sensitive information by viewing or submitting home-care content. We do not intend for advertising or analytics technologies to receive the substance of care inquiries, diagnoses, medication information, service-plan details, or other sensitive client information.

Where required by law or platform configuration, visitors may be offered choices regarding non-essential cookies. Browser controls may also permit users to block or delete cookies. Blocking certain cookies may affect site functionality.

1.7 Text Messages, Email, and Telephone Communications

If you provide a telephone number or email address, Care Concierge may use it to respond to your request and to send communications related to inquiries, appointments, services, scheduling, account administration, or other requested interactions. Optional promotional or marketing communications will be sent only as permitted by applicable law and consent requirements. Consent to receive marketing messages is not a condition of purchasing home care services. Message and data rates may apply to text messages. You may opt out of marketing texts by replying STOP and may request help by replying HELP, subject to the capabilities of the messaging platform. Opting out of marketing does not prevent Care Concierge from sending non-marketing communications that are necessary to respond to your request, administer an active service relationship, or comply with law.

1.8 Data Security

Care Concierge uses reasonable administrative, technical, and physical safeguards appropriate to the sensitivity of the information it maintains. Safeguards may include access controls, authentication, role-based permissions, secure transmission, vendor management, workforce confidentiality requirements, training, backup practices, and incident-response procedures. No website, email system, text platform, or electronic storage method can be guaranteed to be completely secure. Users should avoid sending highly sensitive information through ordinary email or general contact forms unless specifically instructed to use an approved secure method.

1.9 Data Retention

Care Concierge retains information for as long as reasonably necessary for the purpose for which it was collected, to provide or document services, to resolve disputes, to enforce agreements, and to meet legal, licensing, tax, insurance, employment, and regulatory obligations. For individuals who become clients, Georgia Private Home Care Provider rules require client records to be retained for at least five years from the date of the last service provided. Other categories of records may have different retention periods.

1.10 Data Breach and Incident Response

If Care Concierge discovers unauthorized access to information, it will investigate and take reasonable steps to contain and remediate the incident. Where notification is legally required, Care Concierge will provide notice in accordance with applicable law. Georgia law requires certain businesses maintaining unencrypted computerized personal information to notify affected Georgia residents of qualifying security breaches in the most expedient time possible and without unreasonable delay, subject to permitted law-enforcement delay. HIPAA or the FTC Health Breach Notification Rule may impose additional duties when applicable.

1.11 Your Choices and Requests

You may contact us to request correction of contact information, ask questions about our privacy practices, update communication preferences, or request access to information where a right of access applies. Some information cannot be deleted or altered when Care Concierge is legally required to retain an accurate client, billing, employment, licensing, or business record. Rights regarding protected health information, if HIPAA applies, will be administered in accordance with HIPAA and any applicable Notice of Privacy Practices.

1.12 Children's Privacy

Care Concierge's current home care services are intended for adults age 18 and older, and the website is not directed to children under 13. We do not knowingly solicit personal information online from children under 13. If we learn that information was submitted directly by a child under 13 without appropriate authorization, we will take reasonable steps to address it.

1.13 Third-Party Websites and Services

The website may link to or integrate with third-party services. Their privacy practices are governed by their own policies. Care Concierge is not responsible for independent third-party websites or services, although we seek to use vendors appropriate for the information and functions involved.

1.14 Changes to This Privacy Policy

We may update this Privacy Policy to reflect changes in our services, technology, vendors, payer relationships, or legal requirements. The revised policy will be posted with an updated effective date. Material changes may be communicated through additional methods when appropriate or required.
